Critical illness insurance pays a cash benefit for certain policy-defined serious illnesses like stroke, cancer, heart attack and organ failure. As a medical plan participant, you automatically receive $5,000 of critical illness coverage for both yourself and your spouse or domestic partner (if also enrolled). Enrolled child(ren) are also covered at $5,000. This benefit is available to you at no cost from your employer. If you are diagnosed with a covered illness, benefits are paid directly to you upon submission and approval of claim.

Such coverage provides added peace of mind. You may purchase $10,000 or $20,000 of additional critical illness insurance each for yourself and your spouse (your spouse is eligible for coverage only if you enroll yourself). The spouse amount elected should be the same as the amount you elected.
WELLNESS BENEFIT
Your enhanced critical illness benefit also includes a Wellness Benefit that allows you to receive $50 for each covered family member that completes an eligible health screening test.

Yes! Eligible children from live birth to age 26 are automatically covered at no extra cost. Their coverage amount is 100% of yours, and they are covered for all the same illnesses as well as these specific childhood conditions: cerebral palsy, congenital birth defects, cystic fibrosis and Down syndrome. The diagnosis must occur after the child’s coverage effective date. Disabled children enrolled in the plan prior to their 26th birthday may remain on the plan after the age of 26.
